Do you have a chronic health condition and/or are currently on an NHS waiting list?

You can now self-refer to WISE and receive personal support from a Wellness Coach.

The Wellness Improvement Service (WISE) is a wellness coach-led service to empower patients to improve their own long-term health and reduce symptom burden to improve quality of life.

The Wellness Coach helps the patient to look holistically at all the factors affecting their health and happiness with a focus on pain management, sleep hygiene, mental health and social connectivity, physical exercise and following a healthy diet.

Patients then develop tools to overcome some of the hurdles faced when it comes to lifestyle and behavioural factors that impact their health.

You can self-refer to the WISE service here: https://ctmuhb.nhs.wales/wise-ctm/
